Non-Discrimination Notice

Apopka Health and Rehabilitation Center does not exclude, deny benefits to, or otherwise discriminate against any person based on the following protected categories:

2. Providing Equal Access and Accommodations

We are committed to ensuring that all individuals, including those with disabilities or limited English proficiency (LEP), have access to the care and services they need. This includes offering:

  • Free Aids and Services for People with Disabilities:

    • Qualified sign language interpreters.

    • Written information in other formats (large print, braille, audio, accessible electronic formats, etc.).

  • Free Language Assistance Services:

    • Qualified interpreters for people whose primary language is not English.

    • Information translated into other languages.

3. Filing a Grievance

If you believe that Apopka Health and Rehabilitation Center has failed to provide services or discriminated in any way on the basis of race, color, national origin, age, disability, or sex, you have the right to file a grievance. You may file a grievance with our Civil Rights Coordinator:

4. Filing a Civil Rights Complaint with the U.S. Department of Health and Human Services (HHS)

If you believe that we have discriminated against you or failed to meet our non-discrimination obligations, you may also file a civil rights complaint with the U.S. Department of Health and Human Services (HHS), Office for Civil Rights (OCR):

5. Non-Retaliation Policy

Apopka Health and Rehabilitation Center strictly prohibits retaliation against anyone who opposes discrimination, files a grievance, or participates in the investigation of a grievance. We are committed to addressing all concerns in a fair and timely manner, ensuring that individuals are not penalized for asserting their rights under this Non-Discrimination Policy.
